Interagency Links Process
Interagency Links
referrals are for children and students who will require support from
other agencies and services during transition from hospital to school.
These children and students can be identified at admission, through discussion
at the Reflect and Review meeting or through negotiation with the manager
of the Hospital Education Services.
- The Maintaining
of documentation:
- Ensures interagency links forms are completed
- Records notes from teachers, schools, case meetings and from discharge meetings
- Cross checks information from ward rounds with appropriate teacher eg. Oncology, Boylan - Liaises with teacher (case allocation)
- Liaises with manager relating to issues at a school principal and district office level
- Provides consent to link with support services
